LECTURE: External Cervical Resorption: Individualized Treatment in Complex Clinical Cases and the Expanding Role of Intentional Replantation.
Dr. Mile Churlinov is an award-winning endodontist and dental traumatology specialist with extensive experience in microsurgical techniques and tooth autotransplantation. He holds a Master’s in Endodontics from the Universitat Internacional de Catalunya, where he now lectures in postgraduate programs, both in Barcelona and London Dental Institute.
Dr. Churlinov is a recognized international speaker, having presented at major events including the ESE Biennial Congress, Roots Summit, and the World Dental Federation. His clinical and academic work focuses on topics such as pulpotomy in permanent dentition, endo-restorative synergy, and external cervical resorption management. He has contributed to respected publications and serves as a jury member in international scientific competitions. A contributor to “Tooth Autotransplantation in the Digital Era,” he is also an active member of IADT and a Coltene opinion leader.
LECTURE
External cervical resorption (ECR) remains a significant clinical challenge because of its highly variable presentation and the lack of a universally accepted treatment algorithm. This clinically oriented presentation provides a concise overview of the current understanding of ECR, including its etiology, pathogenesis, diagnosis, classification, treatment strategies, prognosis, and long-term follow-up.
Treatment decisions should not be based solely on the extent and location of the resorptive defect. Clinical symptoms represent a key factor in the decision-making process and may substantially influence treatment selection. Furthermore, contrary to common assumptions, the loss of epithelial and connective tissue attachment associated with gingival recession may improve access to the resorptive lesion, thereby expanding rather than limiting the available treatment options.
The presentation focuses on complex symptomatic cases that have traditionally been considered candidates for extraction because of technical limitations associated with conventional treatment approaches. Particular emphasis will be placed on intentional replantation as a tooth-preserving treatment option in carefully selected cases where predictable outcomes cannot be achieved using conventional surgical or nonsurgical techniques. The indications, surgical protocol, advantages, limitations, and prognosis of intentional replantation will be discussed within the framework of a comprehensive treatment strategy integrating restorative, periodontal, and endodontic management.
Using representative clinical cases, this presentation demonstrates how symptom-based assessment, individualized treatment planning, and contemporary therapeutic techniques can broaden the possibilities for tooth preservation and challenge traditional concepts in the management of advanced external cervical resorption.
